What should I bring to my first appointment?
Please bring a completed copy of this new patient form along with your insurance card(s). If you wish to complete the form on the day of your visit, please arrive 15 minutes early.
What insurance plans do you accept?
We accept all major dental insurance plans including Delta Dental, Physicians Plus, American Dental Plan of WI, and WEA. For more information about accepted plans, please call 846-3302. Sorry, we do not honor discount dental program cards.
Do you have any payment plans?
For your convenience, we accept Visa, Mastercard, and Discover. We also offer financing through American General Financing. Financing allows you to make affordable payments on treatments not covered by insurance.
What can you do to ease my fear of the dentist?
If it's fear because you haven't been to the dentist in a long time, we'd like you to know that we are here simply to improve your dental health, and not to give lectures. We can assure you that you are in the hands of a great team of caring professionals. Our patients agree! Please read our testimonials page for their recommendations.
How old should my child be when I bring him for his first visit?
3 years is a generally good age for your child's first check up.
Is it okay to get a second opinion about proposed dental care?
Absolutely. We want you to be in charge of your dental health. It's important that you research your treatment options so that you can make an informed decision. When proposing complex treatments to our patients, we will recommend that they seek a second opinion from another provider. We also provide second opinions and will happily discuss your case with you at no cost.
